New Clients

Finding the Office:
The address is on Redwood Rd, but the office building complex is actually located on 5680 S just off of Redwood. It is behind the large Workforce Services building (which is 5735 S Redwood Rd, on the East side of the street). To get to the complex, turn East on 5680 S as though going to the Workforce Services parking lot and the complex will be visible just past that parking lot. Building 16 is on the South side of the office complex. The door has a list of providers and says “Ariel Gonzales” at the bottom. Please give yourself a few extra minutes to find the building if you have never been to this complex before. Suite 1B is upstairs on the right. (If stairs are a problem for you, just let us know so we can schedule our ground-floor room for your appointments!) Please knock to announce that you have arrived, then wait in the chairs by the door. If it is more than five minutes after your appointment time and you have not been called in, please knock until there is a response.

New Patient Packet:
All clients arriving for a first visit must bring the following packet, completely filled out. If the packet is not 100% complete before the start of the scheduled appointment time, the client will be rescheduled to the next available new patient appointment. This is usually a multiple-week wait so it is VERY important to have the packet completely done before arriving. It’s usually a good idea to complete the packet a few days in advance to ensure you don’t run out of time.

If the client is under 18, be sure to read the next section of this page, where you will find additional instructions and an additional required form.

For clients under 18:
On the above new client packet, please write “N/A” for all questions that don’t apply to people the client’s age. Teens 16 and up can usually complete the packet mostly by themselves, with adult help needed for a few of the questions. Teens 13-15 usually need help with many questions on the packet. For children under 13, an adult usually completes the packet for the child. The adult should write answers from the child’s perspective: on a question like “how have you felt this week?” the adult should answer how the child felt this week. If the adult is unsure they can ask the child.

For all clients under 18, the first page of the packet must be signed by a parent/guardian and that parent/guardian must accompany the client to the first visit. The parent/guardian must also complete and bring the following form. The client should not complete this form even if he or she is old enough to do it. There are two versions to choose from depending on age.
